Number 4 exhaust at least, looks to have been running with zero clearance.
How do the shells in the cam caps look?

What does the micrometer say about the diameter of those bearing journals?
Mine look infinitely worse and i just polished them with solvol to remove the roughness. Not had a single problem with that as they still measured within spec and the oil supply side is very very good and it gets changed regularly. I put new shells in (repro ones) and thats where the problems arose for me.
With regard to the base circle on the number 4 exhaust lobe; again no issue with that as it should never (never again) contact the bucket. I would be more concerned with the lift and lower ramps on the lobe itself! If that is grooved then i would say you are done with them.
